I have been tryin to get a screeny of this problem with source and i finally did. It doesnt happen often but it does happen. Notice in the first pic that there is blood on his right chest and he has full health. I was sure i shot him. The other pics you can see that he was also shot in the hand. I am not saying he is cheating. I have seen this before. I was lucky that he went afk right after he hit me. The bullets registered to make blood but gave no dmg.

it shouldnt be the interpolate. You should just go with cl_ragdoll_physics_enable , and that should solve your problem.
